This portion is paid to the Primary Care Physician (PCP) and Specialist Physician. Depending on the co-pay amount, there may be a difference for either kind of visit. Urgent care and Emergency co-pays are common and typically required at time of visit.

Portion paid for a formulary that is traditionally broken up into tier levels: 1/2/3/4/5. Most carriers have preferred pharmacies and mail-order options. Scripts are usually 30-90 days with the higher tier levels requiring pre-authorization and not offered as a 90 day supply.

This is a combined cost share after the deductible has been met and usually paid on a percentage.

What kind of health insurance will I need?

Health insurance comes in all shapes in sized and if one is not careful they may find themselves with a policy that is inadequate. Knowing basic terminology is important such as “major-medical” because this separates itself from other companies that offer health insurance.

Now, is there anything wrong with other carriers that offer non-major medical coverage? No. It depends on the need. Dental, vision, indemnity, and temporary insurance fall under the category of health insurance but none of them are major-medical.

How do I go about getting a major-medical plan?

First, there is a window of time that anyone can buy a policy which is called the Open-Enrollment Period (OEP). At this time, individuals may apply for a tax-credit and shop on/off the marketplace exchange. Once that period is over, there must be a qualifying life event (QLE) for a Special Enrollment Period (SEP). A QLE would be a job loss, move, marriage, divorce, pregnancy, or involuntary loss of coverage. This is your “golden-ticket” to find another plan.
